Housing Stabilization Services is a Medical Assistance benefit to help people with disabilities, facing homelessness, and seniors find and keep housing. Housing stabilization services are state plan (not waiver) Home and Community-Based Services (HCBS).

Housing Consultation
We assist individuals in obtaining the necessary documentation for Housing Stabilization Services eligibility and work with them to develop a housing-focused, person-centered plan. This plan is tailored to their strengths, needs, preferences, and cultural considerations, helping us find housing that best meets their priorities. Additionally, our housing consultation service empowers individuals to choose their housing transition or sustaining provider and provides resources for services that address non-housing-related goals.
Housing Transition
We assist individuals by helping them develop a personalized housing transition plan and supporting them in applying for benefits to make housing more affordable, including determining which benefits they may be eligible for. We also guide them through the housing search and application process, assist with tenant screening and housing assessments, and ensure that their new living arrangement is safe and ready for move-in. Additionally, we help secure funding for deposits or essential home goods and coordinate their move.

ICS services
Our Independent Supportive Housing (ICS) program is designed for adults with disabilities, focusing on helping clients maintain independence in their home and community. The ICS program is client-centered, providing services that align with individual goals to support their ability to live independently within the community.
Delivered in an apartment setting, each client has their own private unit, complete with a kitchen, bathroom, and bedroom. ICS services can be available up to 24 hours a day, if necessary, and additional support, such as nursing and home care, can be coordinated based on individual needs.
At the heart of our supportive housing program is a commitment to creating a safe, comfortable, and empowering environment. We recognize the importance of having a place to call home, offering clean, well-maintained apartments that clients can truly make their own.
CRS / Adult Foster Care
We offer 24-hour Customized Living services for adults with physical, developmental and/or mental disabilities in a welcoming residential environment. Our aim is to help individuals maintain their independence while fostering their integration into the broader community.
Our services include assistance with bathing, dressing, personal hygiene, grooming, and other daily living activities, as well as any necessary medication management.
We strive to promote the highest level of independence for our clients while providing the support they need.
Home-Based Services
Home-Based Services provide personalized support to individuals in the comfort of their own homes. These services are designed to assist with daily living activities, such as :
- Assistance with dressing, Self-feeding, oral hygiene,hair care, grooming, toileting, and bathing
- Standby assistance within arm’s reach for safety while performing daily activities
- Verbal or visual reminders to take regularly scheduled medication (includes bringing clients previously set-up medication, medication in original containers, or liquid or food to accompany the medication)
- Preparing modified diets ordered by a licensed health professional
- Laundry
- Housekeeping/other household chores
- Meal preparation
- Shopping
By delivering care in a familiar environment, we aim to promote independence and enhance the overall quality of life for individuals with varying needs, including those with disabilities or chronic health conditions. Our dedicated staff works collaboratively with clients and their families to develop tailored plans that address specific needs and preferences.
